Please notify the Practice of any change of address or telephone number.  This is to ensure that your records are up to date, and we can contact you easily.

You can inform us of a change in your personal details through submitting an online administrative query to our reception team here.

If you are moving to an address which is out of our area, we kindly ask that you register with a Practice closer to your new address.

Patients who move from an address outside the catchment area to another address which is also outside the catchment area (such as moving from a Cheadle address to another address in Cheadle) will be asked to register with another GP in the area in which they are now living.
