Proxy Access
Proxy access to online services allows a parent, family member, or caregiver to act on behalf of the patient with customized access. The proxy can easily book appointments, make repeat medication requests, send messages, and access medical records (where applicable) once they are linked to the patient.
If you want to apply for proxy access, you need to personally provide your identification (ID). Additionally, you must have a legal basis for the proxy access. If the person you wish to act on behalf of is over 11 years old, their consent for proxy access must be provided to the practice. In case they are over 11 years old but not mentally competent to provide consent, for example, if they have dementia, the practice will need to verify that you are the appropriate person to act on their behalf as a proxy.
